Mobile Handset Acknowledgement Form
Terms of Use:
This device has been supplied to you in accordance with the Company’s Mobile Device Policy which is
detailed below and we ask that all staff who have a Company mobile device to familiarise themselves
with this policy:
- You may be responsible for the payment of any insurance excess if your device is damaged or
lost.
- If your device is lost or stolen, you must report this as soon as possible to Paul Beckett (or a
member of his team) who will notify the network provider and arrange for the connection to be
stopped. If your phone is stolen, you will also be required to report the matter to the Police and
obtain a crime reference number. You will receive another device as soon as it is practically
possible.
- Please report all instances of damage to Paul Beckett or a member of his team as soon as
possible in order that a repair can be arranged. You will receive another device as soon as it is
practically possible.
- It is strictly prohibited for staff to enter competitions via a premium rate number (voice or text),
or make donations via text from any Company device, and we reserve the right to recover any
monies incurred by these actions from your salary or other monies due to you.
- All Company mobile devices currently have Vodafone EU passports attached to them which
allows you to use your device within Europe up to the same limits as you would in the UK. If you
are travelling outside of Europe, contact IT to discuss this prior to your departure. When using
your mobile abroad, please make the best use of WIFI facilities. We reserve the right to review
this should the Company change networks or there be a change in legislation.
- As with all Company devices, mobile handsets must not be used to access or forward indecent /
inappropriate content.
- Whilst you can use all of the features on your device, if you are storing photographs or any other
types of media on the device, it is your responsibility to back these up. Similarly if you are
storing contacts on the device and not in your Microsoft Exchange account, it is your
responsibility to back these up also.
- You must ensure your device is kept secure at all times, especially when outside the office.
- It is not Company policy to allow members of staff to port their own personal mobile number
onto a Company device.
- Screen locks and passwords must be used at all times to ensure confidential data is protected in the event of loss or theft.
- The Company reserves the right to recover insurance excesses, excess data usage charges,
charges incurred by the use of premium rate services, chargeable apps or similar non business
related usage costs, etc from your salary or other monies due to you.
- HM3 Legal reserves the right to amend this policy and / or remove the usage of Company
mobile phones at any time.
